In 4) shouldn't chloroform and KOH be together? That's the carbylamine reaction...because sodium azide degrades CH3COCl to CH3NH2(Curtius reaction). Heat will cause the isocyanide to isomerise into CH3CN, hydrolysis of which gives carboxylic acid.
